The application process evaluates your credentials, provision of care, facilities, and scope of service.

Member Location Minimum Qualifications:

  • No physician ownership
  • Facility wheelchair accessible and open full time
  • Operational for more than 12 months
  • Schedule three or fewer patients per hour/per therapist
  • Have a comprehensive operational policy and procedure manual
  • Clinic director with three or more years of experience

All of us at Hospital for Special Surgery look forward to working with a high-quality network of therapy providers. 

 The following are required for submission of your application:

  • Proof of license for all therapists, assistants, and athletic trainers (or registration for NY state).
  • Proof of APTA/AOTA or other appropriate professional membership for owners and therapists (preferred).
  • CEU certificates for the past year for all owners.
  • Proof of facility general insurance coverage, showing expiration date.
  • Proof of malpractice coverage and amount of premium paid.
  • Evidence of Medicare certification (if participating).
  • Resumes for all treating Staff members.
  • A copy of all required Policies/Procedures  **Please submit only the required policies, not your entire manual.**
  • $600 non-refundable application and site visit fee
